*BACKGROUND*
The Vaughan Community Health Centre (VCHC) is a not-for-profit, community-governed organization providing primary health care, social services, and illness prevention programs primarily to the residents of the Western and Northern York Region. We serve people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. We are committed to providing services with and for local Indigenous, Black, racialized and other disadvantaged communities.
VCHC has implemented a direct referral system to improve access to inter-professional care by allowing non-team-based family physicians to connect vulnerable (medically and socially complex) clients to services available at the VCHC. As part of the health promotion framework of our services, we work in partnership with other community-based health and social service organizations to address individual, family and community needs.
*POSITION SUMMARY*
As a member of the Primary Health Care Team, the Nurse Practitioner is responsible for the provision of primary health care services in accordance with the College of Nurses of Ontario Standards of Practice for RN (EC)s. The position has a strong emphasis on health promotion and disease prevention and works within a social determinants of health framework.
*KEY ACCOUNTABILITIES AND RESPONSIBILITIES*
* Provides primary health care services to all ages.
* Acts within the scope of the RN (EC) which includes but is not limited to: history-taking, advanced physical assessments, diagnostic reasoning and development of treatment plans including health promotion/education/counselling, screening, referral, and follow up.
* Consults with physicians and/or other health care providers when client's condition requires care beyond the scope of a RN (EC)?s practice.
* Provides services during regular and extended hours on a scheduled basis and on urgent basis in a variety of settings such as: within the clinic, in community locations, satellite offices, and home visits. Possible sharing of on-call duties with other providers.
* Works within a health care framework which emphasizes health promotion and
* education, disease prevention, and recognizes the special risk factors (social, physical, psychological, etc.) which may affect the health of clients.
* Works in partnership with other members of primary care to share responsibility for planning, provision and coordination of client care.
* Ensures appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussions of problem cases and answering patient inquiries and directing them to the appropriate treatment/resource.
* Supports and monitors VCHC?s advanced access model by working closely with the Reception Team Lead and the Primary Healthcare Manager to ensure that: (a) supply and demand status is kept balanced and (b) the Third Next Available Appointment (TNA) is kept as close as possible to the 0-3 target established by VCHC.
* Works with the Leadership, Reception Team and Primary Health Care Teams to ensure that VCHC?s panel size and clinical targets which are identified in the Multi-sector Accountability Agreement (MSAA) are met. When there are no circumstances affecting intake, the Nurse Practitioner will be responsible to achieve the panel size.
* Be familiar with VCHC?s high risk policies and implement them in daily practice and care for clients and their families.
*QUALIFICATION REQUIREMENTS*
* Masters prepared RN (EC) mandatory
* Active registration and good standing with the College of Nurses of Ontario (CNO) as a Primary
