You will achieve this through the investigation and assessment of clients referred, as well as by working closely with other departments, stakeholders, care coordinators, health professionals and other community agencies. </div></div><div style="padding:10.0px 0.0px;border:1.0px solid transparent"><div style="font-size:16.0px;
