They will integrate meaningful participation for clients and develop complex case plans to prove overall comprehensive support, referrals, and crisis interventions while fostering community partnerships and ensuring the effective use of resources.</p><p><strong> </strong></p><p><strong>Job Responsibilities:</strong></p><ul><li>Conduct Harm Reduction needs assessments and provide coordination, planning, implementation and evaluation of data collected from multiple sources.</li><li>Educate and train staff around Harm Reduction guidelines, procedures, and standards to promote consistent service and to ensure effective use of resources.</li><li>Supervise staff on a team.</li><li>Educate staff on effective practices and referrals for clients surrounding health and harm reduction supports, resources, and techniques.</li><li>Develop and monitor Harm Reduction supplies and inventory management systems at Homes First sites.</li><li>Assist in the development and maintenance of Harm Reduction and health partnerships.</li><li>Development and implementation of new harm reduction and health programming.</li><li>Serve as a central point of contact for Harm Reduction and health questions and concerns within Homes First.</li><li>Support peer worker involvement in planning, implementation, and evaluation of harm reduction initiatives.</li><li>Provide meaningful participation of clients who use drugs in the creation of programming that serves them.</li><li>Develop and assist staff in execution of complex case plans for clients with mental health, addictions and physical health needs with a particular focus on those in crisis, including if necessary, interventions involving the Mental Health Act, MCIT, and Form 2s.</li><li>Liaise with hospitals while clients are admitted including collaborating in discharge planning.</li><li>Provide referrals, programming, support services, rehabilitation opportunities, education, and addictions treatment.</li><li>Support and direct the ICM Team and other staff with making follow-up phone calls to clients.</li><li>Utilize knowledge of issues about health care, mental health, addictions, homelessness, employment, education, internal policies and procedures to develop program materials and partnerships.</li><li>Develop case plans, utilize case management tools, crisis prevention and harm reduction interventions, report case notes, and develop a strategy for clients within caseload, providing case management, care coordination, and episodic/crisis/referral across the Greater Toronto Area.</li><li>Collaborate and support interventions, safety plans, safe exits, and access to emergency services for program participants.</li><li>Ascertain and provide participants with referral services within the community and/or social services resources.</li><li>Assist Management in establishing meaningful connections with community partners, support and participate in
